Please attach your request for renewal (either PDF or Word). Please include the name of the college or university you will be attending for Fall 2025 and your student ID number in your document.
In order to continue to receive your scholarship each year, you must submit your most recent unofficial transcript. Continued eligibility includes evidence that you maintained a 3.00 grade point average on a 4.00 scale.
In order to continue eligibility for the scholarship, you must also submit proof of continued enrollment as a full-time student in a two-year associates or four-year undergraduate degree program at an accredited college or university. Please upload your next semester course schedule below.
